About Chris Bratcher
Chris Bratcher, APRN, is a Nurse Practitioner with a strong background in delivering comprehensive, patient-centered care across a wide range of clinical settings. He currently works within an Accountable Care Organization (ACO) program serving Medicare patients, where he focuses on chronic disease management, preventive care, and care coordination to improve patient outcomes. He also supports podiatric care with Upperline Health, assisting patients with complex foot and lower-extremity conditions as part of a multidisciplinary healthcare team.
Previously, Chris practiced at Rapid Relief Urgent Care, where he provided timely, high-quality acute care in a fast-paced environment, strengthening his clinical decision-making skills while maintaining a compassionate, patient-focused approach. Earlier in his career, he worked with Advanced Care House Calls, delivering primary care services directly in patients’ homes. This experience fostered a deep appreciation for individualized care, particularly for patients with limited mobility or complex medical needs, and reinforced the importance of collaboration with patients, families, and caregivers.
Chris’s clinical specialties include chronic disease management, preventive care, and care coordination for Medicare and medically complex patient populations. He brings experience in primary care, urgent care, home-based medicine, and specialty support, with an emphasis on interdisciplinary collaboration, risk reduction, and continuity of care.
Outside of clinical practice, Chris remains actively involved in his community as an assistant coach for a high school wrestling team, where he enjoys mentoring student-athletes. He stays active through running events and weightlifting and enjoys gardening, reading, and spending time with his animals, including his dogs and cat. He values balance and personal growth and strives to bring the same dedication and care to his professional and personal life.
